Aneurysmal bone cyst is a nonneoplastic lesion of long bone, usually occurring in skeletally immature individuals. Radiographically, aneurismal bone cyst is an eccentric expansile lesion commonly located at the metaphysic of long bones. Aneurysmal bone cyst of the long bones in a purely intracortical location is unusual. A 46-year-old woman presented with a spontaneous left lower leg pain. Conventional radiographs of the left tibia showed a 4x1.8 cm eccentric lytic lesion at the cortex. The lesion had relatively well-defined margins. On T1-weighted MRI sequences, the lesion exhibited low signal intensity. On T2-weighted MRI sequences, the lesion showed high signal intensity. There was no cortical destruction or soft tissue abnormality. For the treatment, curettage of the lesion was performed. Histologically, the lesion was composed of benign reactive tissue with a fibrous stroma and reactive bone surrounding blood-filled spaces. Here we report an unusual case of intracortical aneurismal bone cyst of the tibia.
